NEW PAWEST Disclosure Requirements for ALL Coaches
Starting with the spring season for 2008:
ALL coaches must have completed an online risk management form.
This is similar to an Act 33/34 clearance process. The cost is $10 per coach (the
clearance is good for 2 years) - you can either pay by credit card online, or send a check to PaWest.
The
process is really fairly simple - just access by going to www.Pawest-soccer.org

and self-explanatory "Kidsafe Disclosure Application".
The information you provide is on a secure
site. Hampton Soccer Club does not have access to your personal information. We are only able to view a site that shows
whether you have received clearance. This is done through a national clearance organization.
Because there is
a $10, we have devised a method to reimburse you. We would like to avoid sending out $10 checks to the coaches. Therefore,
for the spring 2008 season, we will deduct $10 from one application for one of the children you register. This may be
done at the 'Live' registration, or there will be instructions on the new, soon-to-be-launched online form.
COMMISSIONERS
- this is an ABSOLUTE must for each coach; otherwise, they will NOT be permitted to coach in the spring.
